Please consult the Play Well Affiliate Grant Guidelines for a full definition of eligible and ineligible organisations and more information on the grant requirements. 

1. Are you a member of the Play Well Affiliate Program with a valid Program Agreement and listed on the Play Well Affiliate Program webpage? * Required

2. Has your organisation or any proposed partner organisations, been named by the National Redress Scheme for Institutional Child Sexual Abuse as "Declined to join"? * Required
Response required.
3. At the time of submission, does your organisation have any outstanding debts through other funding programs or services administered by the ASC? * Required

4. At the time of submission, has your organisation successfully met all reporting requirements of previous Play Well funding programs administered by the ASC? * Required

Programs include the 2024/25 Play Well Affiliate Grant and 2024 Play Well Participation Grant
